Our Direct Primary Care (DPC) model is designed to offer high-quality, personalized healthcare without the complexities of insurance. Under our DPC model our providers offer services such as annual physical exams, wellness visits, medication management, preventative care, sports physicals, work physicals, management of chronic conditions such as high blood pressure, heart failure, diabetes and treatment for acute illnesses such as cold, ear infections, rashes, suture removal among other medical concerns. Our services are available for women and men ages 18 years and older.
When participating in our Direct Primary Care (DPC) service, we require initial visits to be completed in the office. Following the initial visit, follow-up appointments may be completed either in person or by telehealth at the preference of our patients and as medically appropriate. Once established with our office, patients have direct access to our providers through text, email and phone for direct support. Our goal is to provide timely and effective care to our patients, to reduce wait times to address medical concerns and improve the health and wellness of the patients we serve.
In order to provide cost effective care, our Direct Primary Care service operates on a flat affordable monthly fee. We do not accept insurance and cannot accept HSA as a form of payment. The monthly fee is payable by cash, credit or debit. This allows for straight forward billing with increase transparency for our patients while avoiding the hidden fees, co-pays and high deductible plans that are often associated with insurance. There are no contracts or cancellation fees, allowing patients the flexibility to cancel their membership at any time.

Customizable Pricing: Here at Direct Primary Care, LLC we provide a range of services beyond our Direct Primary Care. Our patients may access as many or as few of these services as they desire. Some of these services are associated with additional costs which will be discussed openly and in plain language at time of inquiry. Patients are never left to guess what the cost of their care will be or wondering if there will be additional fees or bills.
Personalized Attention: Our clinicians manage a small patient panel, ensuring more personalized and focused care allowing us to respond to the needs and concerns of our patients quickly thereby reducing wait times.
Extended Consultation Time: Unlike traditional primary care, our visits are never rushed and may last at least 30 minutes. This move away from the traditional 15 minute visits affords our providers the opportunity to develop meaningful relationships with our patients.
Flexibility of Visits: Patients can schedule as many appointments a month as needed without additional charges beyond the affordable flat monthly fee. These visits may be in person or through telehealth. Additionally, our flat monthly fee covers direct phone, text message and email access to our clinicians.
Comprehensive Care: By working closely with our patients, we hope to develop strong provider-patient relationships that lead to long-term relationships. In this way we hope to be partners in our patient’s journeys to achieving improved health and wellness.
Prompt Response: Traditional primary care offices require each clinician to care for a panel of approximately 2000 patients. Here at Premier Health Care, LLC our vision is one where our clinicians carry a panel of no more than 500-600 patients. This allows our clinicians to quickly address the healthcare needs of our patients with long visits and more individualized care.
